Student Vaccination Preregistration Forms

Dear parents and guardians,

Our student vaccination clinic will be run by Rite Aid through the regional health network in our area and our fire department. To get started, we are required to pre-register all students. All fields in the preregistration form must be completed to be eligible for the vaccination. Please note that you will need to fill out a separate form for each child in your household age 12 and up that you wish to have vaccinated at our clinic.

In short, complete a preregistration form for each eligible child by Tuesday, May18th.

You will receive a confirmation email from Patty Banfield with an appointment time and further instructions no later than Thursday, May 20^th.
